Building a Lesson Plan Feedback form
Please note: this article is primarily for users with a System Admin role attempting to build a Lesson Plan Feedback form.

Getting Started
Click Config > Lesson Plan Forms1
Select Add Lesson Plan Form
Choose a name for your form
Form Info
The Form Info section allows you to manage the behavior of your Lesson Plan Feedback Form. From here, you can choose which Document Types can be submitted for this form if you've configured Document Submission Types in your site's Metadata. You'll also be able to exclude this form from certain Schools or Roles, for example, if this form is only meant to be used by a specific group of users.
Form Builder
Start by adding a Section and then a Field to your form. Each Section and Field has a Name and Description option to make it easier for Lesson Plan reviewers to understand the purpose of a Section or Field. You'll also have the option to Hide individual Fields from the submitter.
| Field Type | Description |
| Short Text Box | A text box that fits one line of text and cannot be expanded |
| Standard Text Box | A text box that fits a paragraph and can be expanded |
| Long Text Box | A text box that fits a few paragraphs and can be expanded |
| Checkbox | An individual checkbox with a label used for true/false inputs |
| Dropdown | Standard dropdown with option to choose placeholder |
| Help Text | A uneditable text box for LPF form builders to write helpful information for LP reviewers |
